Communication Is Key

One of the most important aspects of disease management is communicating with your providers and caregivers. This can easily become complicated to track and manage when you are referred to specialists, sent for testing, or prescribed new medications. As part of our medical care service, we help track all of the paperwork, and make sure your health information gets to the right people at the right time — including designated family members and caregivers. We also help with coordinate your care and follow-up appointments so you can focus on getting healthy instead of aligning schedules and making phone calls.

The Three Rs: Reminders, Refills, and Reports

It is important to take your medication as prescribed. To help you follow the doctor’s orders, we setup medication reminders that work for you — whether that means using paper and a pen, or technology like Amazon’s Alexa, Google Home, or Apple’s Siri. In addition to reminders to take your medication, we also track how many doses you have left and assist with getting refills before you run out.

When it’s time to visit your doctor for a check-up, we’ll provide them with a health report so you don’t have to worry about forgetting things during your appointment. Your personalized report will include information such as:

  • Your vitals — weight, blood pressure, blood sugar, etc.
  • Your medication compliance and any side effects you may have experienced
  • What you’ve been eating and drinking
  • How well you move around the house and what tasks, if any, give you trouble
  • Changes in your energy levels, mental health, or appetite
  • Questions you’ve thought of ahead of time
