The phrase “long-term care plan” often makes us think of choosing a nursing home or hospital-like setting for a loved one. However, long-term care planning is more than that. It’s determining how much help is needed now, and deciding between a skilled nursing facility (SNF), assisted living, and home health care. It’s asking your loved one how they feel about end-of-life care, discussing their finances, reading through insurance paperwork, and preparing multiple legal documents.

During your session we’ll discuss multiple aspects of senior care planning.
- The difference between home care, assisted living, and skilled nursing facilities
- The basics of Medicaid and Medicare
- Important legal and financial documents
- How to approach difficult conversations with family
After discussing the primary pieces of elder care planning, the remainder of our time will be dedicated to answering your specific questions, and creating an actionable list of next steps tailored to you and your family.
Please note: In order to make the most of your session, we ask that no more than two people be on the call. If you need help facilitating conversations with other family members, we’re available to assist with that on a separate call after your initial session.
Begin Your Care Planning Journey
- Gather information on your loved one’s health insurance and their current state of health
- Think about your personal goals for your loved one
- Ask your loved one what their needs, wants, and goals are (it’s ok for these to be different from your own)
